Volunteer of Distinction Award Form

The Volunteer of Distinction Award is to honour an individual who has made an exceptional and sustained contribution to Osteoporosis Canada and has demonstrated outstanding leadership in their voluntary capacity. 

Award Criteria

The Volunteer of Distinction Award is presented annually to an active volunteer who: 

  1. Is a current, active volunteer. 
  2. Has served for a minimum of seven (7) years.
  3. Has served in various volunteer capacities with OC as an educator, member of the Board of Directors, Scientific Advisory Council, COPN Executive or in other volunteer roles. 
  4. Has provided outstanding leadership (for example, being a role model to others, motivates volunteers, forges strategic partnerships with other groups/companies). 
  5. Are considered integral to the organization by proving their commitment to the mission and vision of Osteoporosis Canada. 
  6. Has been instrumental in building the capacity of the organization (actively encouraged others to join, spearheaded a new program/ initiative). 
  7. Has furthered the osteoporosis cause on a local, national or international level. 
  8. Acts as a catalyst for enduring change (i.e., moves the organization forward). 
  9. Attach additional supplementary information if desired.

Nomination Submission Criteria

  • A member of the OC community who is a current member, program participant, volunteer or staff person can nominate a volunteer who meets the criteria and who best embodies the award’s spirit using the attached application form.
  • Nominations submitted after the deadline are ineligible.
  • Nominations must be submitted by the deadline of July 15, 2023 at 5:00pm EDT via the online nomination form.
  • All eligible nominations will be reviewed after the submission deadline.
  • Eligible nominations will be judged based solely on the submitted information. Only those individuals who best fit the set criteria will be granted the award. 
  • The Volunteer of Distinction Award will be presented at OC’s Annual General Meeting (AGM), if possible, or at a local recognition event.
  • One Award to be presented per calendar year.
