Backbone Award Form

The Backbone Award is presented annually to active volunteers who:

1. Are a current, active volunteer.
2. Have served for at least one year.
3. Are considered integral to the organization by proving their commitment to the mission and vision of Osteoporosis Canada.
4. Provide leadership, inspiration, or guidance to other volunteers and / or community members.
5. Demonstrate the spirit of volunteerism through their on-going actions.
